Powered
by Epic
Powered by Epic, Acumen 2.0 offers best-in-class EHR
solutions to nephrologists. The solution is designed to meet the needs of
nephrology practices by providing enhanced functionality and improved access to
patient data.
It also provides patients with better healthcare
outcomes. The technology empowers physicians to deliver a comprehensive and
personalized care experience.
The new platform provides an easy-to-use, nephrology-specific
patient portal that enables direct communication between providers and
patients. It includes features previously unavailable to Acumen patient portal
users. It also includes custom reports for CKD patients management.
Acumen 2.0 is also designed to improve interoperability
and scale. This means that providers can communicate and exchange information
with other members of the Epic community, and with hospitals. It also improves
access to patient information by creating an open and collaborative
environment.
Another important feature is the Care Everywhere network.
This allows providers to easily include patient information on claims. This is
particularly important for advanced payment models.
Nephrology-focused
user interfaces and features
Touted as the king of the pack by virtue of their large
number of users, Epic and its ilk have an eye on the patient care front. The
eponymous has a large base of medical professionals to chum with and a large
stake in the ambulatory triumphere. The company boasts a plethora of ambulatory
triumpheres in the form of physicians and allied health care providers. The
triumpheres can be found in many an outlying locale. The company also happens
to be one of the few companies that is capable of coordinating with the likes
of one of their competitors. They have a good foot in the door and a well
defined strategy for the future. Keeping track of your medical staff is a
daunting task but not unnerving if you have the right tools in the right place.
Fresenius
Medical Care North America partnership
Earlier this year, Fresenius Medical Care - North America
announced a groundbreaking partnership with Livongo Health. The company will
leverage the Livongo platform to help chronic kidney disease patients get the
best possible treatment at the best possible time.
This new partnership will enable Fresenius to better
deliver targeted care coordination services, including care management for
multiple chronic conditions, to more than 11,000 kidney patients. It will also
enable Fresenius to expand into the $120 billion chronic kidney disease stage
three to five market. The new company will operate under the InterWell Health
brand.
The partnership focuses on early detection of CKD, which
will increase the likelihood of optimal dialysis starts and, ultimately, reduce
hospitalizations. It also aims to improve quality of life for patients with
kidney diseases.
The partnership will be funded through available cash and
additional debt financing. The deal is expected to close in the second half of
2022.

Using Acumen 2.0 and the MyChart portal, patients can
communicate directly with their physician and nurses. This helps them schedule
appointments and view their medical record. The practice also has access to
information from Epic's Care Everywhere network, allowing RCA to import data
from other Epic providers.
The practice has seen a significant increase in coding
efficiency. It has also noticed that patients have a more accurate view of
their health. In addition, the practice is well-positioned to address the needs
of its patients during the transition to value-based care.
The team continues to make improvements to the product as
time goes by. This includes the addition of nephrology-specific workflow tools.
Among these are the SmartSets, which helped drive consistency and best practice
in nephrology.
The Acumen team has worked closely with the Epic team to
make the product more nephrology-specific. Moreover, the company has optimized
the EHR user interface for nephrology. Lastly, Acumen has created a registry
that allows patients to be enrolled based on diagnosis and clinical parameters.
